Article I
DECLARATION OF POLICY
This policy paper provides guidelines governing the Damayan Benefit. The Damayan Benefit is a financial assistance given to Regular and Associate Members in case of death in the family.
Article II
WHO ARE ENTITLED
All Regular and Associate Members in Good Standing who have been a member of HPPEMC for at least a year and are contributing to the Members Benefit Fund are entitled to the Damayan Benefit. Coverage include the entitled member and his/her dependents up to the 1st degree of affinity and consanguinity (i.e. spouse, children, parents) plus siblings1. Any entitled member of HPPEMC will get P10,000.00 burial assistance in case of death of member or his/her qualified dependent.
Article III
MECHANICS OF DAMAYAN BENEFIT FUND
- Once an entitled member lost a qualified dependent, he/she must fill-up and submit the Damayan Claim Form with all the required attachments.
- Once verified, HPPEMC will release a P10,000.00 Damayan donation to the bereaved member as burial assistance.
- If the entitled member is the one who died, HPPEMC will secure a copy of the death certificate and will release the P10,000.00 Damayan donation to the member's beneficiary.
- A Damayan Benefit Fund will be set up from net surplus or other fund-raising activities of the coop.
- If the Damayan Fund is unable to cover a Damayan claim, the donation will be shouldered by HPPEMC as an expense.
- Member must be in good standing to avail of the Damayan Benefit.
Article IV
DOCUMENTARY REQUIREMENTS
The following are required attachments and must be submitted together with the accomplished Damayan Claim Form. All documents must be submitted before claim can be processed.
- Certified photocopy of Death Certificate
- Birth Certificate of Coop Member (if claim is for deceased parent)
- Birth Certificate of dependent (if claim is for deceased child)
- Marriage Contract (if claim is for deceased spouse)

Notes:
1. For the Damayan Benefit Fund, only the family members up to the 1st Degree of Consanguinity and Affinity plus siblings1 are covered.
2. Step relationships (e.g. step-father, step-sister, etc.) are considered to be the same as blood relationships.
3. Only one (1) Damayan Benefit may be claimed per deceased dependent/member.
